Instruction on Brazilian Visa Application

1) Passport

US citizens: Original (no copies), signed by the applicant, with a minimum remaining validity of six months left and with at least two blank pages left.

If not, please visit our website to Renew or Add Pages to your passport.

Non US citizens:
a) Original (no copies), signed by the applicant, with at least six months remaining validity and with at least two blank visa pages left.
b) Proof of legal residency as a photocopy of green card or of a valid visa in the United States, or an American Re-entry Permit

2) One Visa Application Form
The form must be filled out online. When click "Submit", it will generate an application form with a protocol code on top of it. Click here to fill out the form online. 
 
3) One Photograph
Passport size, in color or black and white, full-face only, on a white or clear background. (We have on site photo service)
 
4) A Copy of Driver's License or State ID

6) Round Trip Ticket
Applicants must present one of the items below:

a)  a copy of the round-trip-ticket, e-ticket or e-booking;
b) OR a signed letter from the airline or travel agent, on their letterhead, with the itinerary, name(s) of the passenger(s), flight number or cruise information, and dates of arrival and departure from Brazil.

NOTE: Airline employees and their dependents must present a copy of their airline I.D. card, and a letter from the airline, on its letterhead, with the itinerary.
 
For Business Only:

7) Letter by Sponsor/Invitation letter
An invitation letter with letterhead from Brazilian Company, detailing person's name, purpose to visit Brazil, Duration of Stay.
 
AND a US Employer with following details:

a) Written in letterhead;
b) Indication, or confirmation (in case of an earlier visa) of the applicant's employment status (whether employee at will, independent contractor, trainee or other);
c) Disclosure of the schedule of the business trip to Brazil, with full details of each activity performed while in Brazil;
d) Ensurance of the company's responsibility for the applicant's expenses and activities whenever he or she is in Brazil on behalf of the company, within the duration of the visa;
e) State the contact person's name;
f ) Signed by a high-ranking official (other than the applicant) entitled to commit the company to the trustworthiness of all of the statements made in the letter
 
8) Fees
Tourist Visa: It is $150/person payable To Brazilian Consulate in Chicago.
Business Visa: It is $210/person payable to Brazilian Consulate in Chicago.

It is $60/person payable to Metropolitan Express for 4-6 workday service fee.
It is $30/person payable to Metropolitan Express for 7-10 workday service fee.

METHOD OF PAYMENT
The Consulate General in Chicago ONLY accepts payments with Postal Money Order issued by the U.S. Postal Service payable to "Consulate General of Brazil in Chicago".
 
Multiple applications submitted together may be covered by one single money order for the total sum.
 
Shipping Method for Returning Passport

A. $20 in additional for Next Day shipping (trackable and insured)

   Or $10 in addition for 2 workday shipping (trackable and insured)

B. Or customers provide by themselves a Self Addressed envelope with postage for return or FedEx or UPS account number. (FedEx, UPS recommended)

PLEASE NOTE
► Minors under 18 years of age: 
• A copy of the minor's birth certificate is required in all cases;
• Minors not traveling with both parents/guardians must provide a notarized letter of consent signed by the non-accompanying parent(s)/guardian(s) authorizing the Consulate to issue a visa;
 
► A International Certificate of Vaccination may be needed for some areas in Brazil.
