2. A completed visa application form Q1 (Download Q1 Form) AND a copy of it.

4. For China Born US citizen, the following documents are needed

    A. If it is the first time to use US passport to apply for a Chinese visa, submit your original Chinese passport together with your US passport. (Both passports will be returned upon approval of the visa.)

    B. OR, if you have a previous Chinese visa, submit the photocopy of your previous Chinese visa together with your US passport.

    C. Chinese name in Chinese Characters is needed on Chinese visa application form.

5. For US born Chinese baby, the following documents are needed

    A. A copy of baby's Birth Certificate is needed

    B. AND a copy of both parents' passports first page with photo and name is needed

6. For Non US citizen, the following documents are needed

    A. Proof of legal status in USA: a copy of US green card, or F1, H1, I-20, etc...

    B. AND a completed supplementary form (download here).
